PATIENT PORTAL
INSTRUCTIONS
Crystal Coast Family Practice is excited to announce our Online Patient Portal. This is a great tool to communicate with our practice! Through this online portal, you are able to complete your registration forms, request an appointment, request refill renewals, pay your bill as well as submit secure questions about your health and treatment to our dedicated staff members.
Signing up is simple. If you have given us your e-mail address, you will receive an e-mail inviting you to register for our Secure Patient Portal. If you have not given us your e-mail address, please use our Contact Us page to send it to us. We will update your information and add your e-mail address, which will generate an invitation to you, so you can register for this convenient service. You'll enter your basic demographic information as well as choose a unique user ID and password. Once you have created an account, you will have access to the following features:
- Health Forms- this allows you to complete and update all paperwork that was previously completed in the office. This will save you time before your appointments. Simply click on the health forms and complete or update your information as need be. Our staff will then have your completed and up to date paperwork before you arrive at the office.
- Appointment Requests- request an appointment with your provider online. You will enter in your preferred day and time and we will coordinate with our schedule. This allows you to schedule an appointment quickly while avoiding waiting on hold for assistance.
- Pay My Bill Online- avoid overdue or late bills by paying your balance online at anytime. Once you have created an account, you will be able to enter your credit card information along with your patient account number to make a payment.
- Prescription Renewal-
request a refill on your current prescriptions. You will just
need the basic information from your prescription label to request a
renewal.

your health care or concerns answered? Use this feature to securely
email us any question you have about your health care or your health
conditions.
